If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. In addition to making your dental experience comfortable, we work very hard to make financing comfortable! We have several ways to finance your dentistry. Give us a call, we’ll help you find the plan that is best for you!
Major credit cards are accepted, as well as personal checks. We also offer an interest-free financing through Care Credit to make payment arrangements more comfortable for you.
